What you can take care of prior to enrolling in Bellevue Public Schools
- Immunizations
- Physical Exam
- required for all students entering kindergarten, 7th grade, and those transferring in from out-of-state
- physical exams can be taken within six months prior to enrollment in Bellevue Public Schools
- parents have up to 30 days following the date of enrollment to present a record of the physical exam
- parents may object to the exam by completing a waiver
- Eye Exam
- required for all kindergarten students and those transferring in from out-of-state
- eye exams can be taken within six months prior to enrollment in Bellevue Public Schools
- parents have up to 30 days following the date of enrollment to present a record of the eye exam
- parents may object to the exam by completing a waiver
- Dental Exam
- recommended for all students entering kindergarten
- Athletic Physical
- for all students participating in sports activities, grades 7-12
- student must have received a sports physical prior to beginning any sports practice
- an athletic physical qualifies as an entrance physical; however, an entrance physical does not qualify as athletic physical
The following information is requested at the time of enrollment
- Certified birth certificate (required)
- Proof of residency (purchase agreement or rental/lease agreement or tax receipt/statement AND utility bill) (required)
- Record of immunizations (required)
- Previous school records
- Other information necessary to support the learning process for your child (i.e. academic, medical, social support services)
- Custody agreements/court orders/restrictions (if applicable)
- Immunizations
