• Enrollment Checklist

    What you can take care of prior to enrolling in Bellevue Public Schools

    • Immunizations 
    • Physical Exam
      • required for all students entering kindergarten, 7th grade, and those transferring in from out-of-state
      • physical exams can be taken within six months prior to enrollment in Bellevue Public Schools
      • parents have up to 30 days following the date of enrollment to present a record of the physical exam
      • parents may object to the exam by completing a waiver
    • Eye Exam
      • required for all kindergarten students and those transferring in from out-of-state
      • eye exams can be taken within six months prior to enrollment in Bellevue Public Schools
      • parents have up to 30 days following the date of enrollment to present a record of the eye exam
      • parents may object to the exam by completing a waiver
    • Dental Exam
      • recommended for all students entering kindergarten
    • Athletic Physical
      • for all students participating in sports activities, grades 7-12
      • student must have received a sports physical prior to beginning any sports practice
      • an athletic physical qualifies as an entrance physical; however, an entrance physical does not qualify as athletic physical

    The following information is requested at the time of enrollment

    • Certified birth certificate (required)
    • Proof of residency (purchase agreement or rental/lease agreement or tax receipt/statement AND utility bill) (required)
    • Record of immunizations (required)
    • Previous school records
    • Other information necessary to support the learning process for your child (i.e. academic, medical, social support services)
    • Custody agreements/court orders/restrictions (if applicable)
