Athletic Physicals

All athletes must have an athletic physical on file before they can check out equipment and participate in practice. Athletic physicals are available at many clinics throughout the Bellevue/Offutt community. Students can download the athletic physical form above or pick one up from their school prior to their doctor's appointment. A parent must be present during the physical to sign the appropriate form.

All portions of the Pre-Participation Physical Examination must be completed and signed by the physician, parent, and athlete in order for the physical to be valid.
