Physicals/Immunizations

Physical Exams:

Nebraska Law requires a physical exam (click here for form) for all students entering kindergarten, seventh grade and those transferring in from out-of-state. The physical exam must have been within the previous six months. Parents have up to 30-days following the date of enrollment to present evidence of the physical exam or object in writing.

Athletic Physical Exams:

Students in grades 7-12 that will be participating in athletics are required to have an athletic physical (click here for form). Athletic physicals count as regular physicals; however, regular physicals will not qualify as athletic physicals.

All athletes must have an athletic physical card on file before they can check out equipment and participate in practice. Athletic physicals are available at many clinics throughout the Bellevue/Offutt community. Students can download the athletic physical form above or pick one up from their school prior to their doctor's appointment. A parent must be present during the physical to sign the appropriate form.

Immunizations:

Nebraska Law requires all students to be immunized against measles, mumps, rubella (MMR-series of two), poliomyelitis, diphtheria, pertussis, tetanus, hepatitis B (series of three), and Varicella (VZV-series of two) or documentation of the Varicella disease, prior to enrollment (click here for summary of school immunizations rules & regulations). Students may be provisionally enrolled if they have begun the required immunizations and continue to receive the immunizations as rapidly as medically feasible, or if the student's parent is on active duty in the military and the child is enrolling in school following residence in another state or country.

Exemptions will be granted for medical or health reasons substantiated by a signed statement from a physician, or religious conflict substantiated by a signed affidavit from the student's legal guardian.

Eye Exams:

Nebraska law requires that all kindergarten students and students entering school for the first time in Nebraska to have an eye exam (click here for form). The exam must be completed by an optometrist, physician, physician's assistant, or advanced practice registered nurse. The exam must be completed within six months prior to the child's entry into school.
